Chronic Disease Reviews

Asthma Reviews can be booked with our Practice nurses. The appointment lasts for around 20 minutes and will involve but not limited to:

• Inhaler technique, management plan
• Review of your inhalers/medication
• Record your exacerbations
• Asthma control test for over 12yrs 
• Peake flow/ Spiro screening 
• General examintion of height, weight etc

COPD reviews can be booked with our Practice nurses. The appointment lasts around 30-40 minutes and will involve but not limited to:

• Inhaler technique, management plan
• Review of inhalers,medication, home oxygen
• Record your exacerbations 
• Respiratory plan/ Spiro screening
• General examination of height, weight etc

Hypertesnion Reviews can be booked with our Practice Nurses or our Health Care Assistant (HCA). The appointment lasts around 20 minutes and will involve but not limited to:

• Blood pressure and pulse rhythm
• General health check height, weight etc
• Exercise and activity
• Includes a blood test 

Diabetes Reviews can be booked with our Practice nurses. The appointment lasts around 30 minutes and will include but not limited to:

• Review of bloods (Hba1C)
• Referral to diabetes programme 
• Diabetes advice, plan, follow up and structured education programme
• Foot exam, urine test
• Medication review 
• General examination of height, weight etc 

Healthy Heart Reviews can be booked with our practice nurse or Health Care Assistant (HCA). The appointment lasts around 20 minutes and will include but is not limited to:

• General examination or height, weight etc
• Blood pressure and pulse rhythm
• Smoking status
• Medication review 
• Exercise and activity
• Diet and weight management advice 

Learning Disability Reviews can be booked with the practice nurse first (30mins) and then an appointment with their allocated GP (10mins) so a total of 2 appointments on the same day. The review will include but is not limited to:

• Discussion around support, immunisations and additional support needs.
• Discussion around mobility, daily living and lifestyle and health promotion.
• GP assessment 
• General examination of height, weight etc
